We are NOT in network with medical insurances
Our practice participates with most major dental insurance plans. Any coinsurance, deductibles and disallowed charges will be due at the time of your visit. After filing your insurance, if there is no or partial payment made by your insurance, you will be responsible for remainder amount.
If we do not participate with your insurance plan, we will gladly file your insurance as a courtesy. You are responsible for all charges to your account, regardless of any arbitrary determination of “usual and customary fees” by your insurance carrier. If insurance has not responded to a claim within 60 days of submission, the full account balance becomes the account holder’s responsibility.
